Hegseth maintains that testosterone replacement therapy will remain optional, distancing the initiative from performance-enhancing drug use. However, the Pentagon has not provided specific research or academic data to support the blanket screening mandate, which contradicts standard medical guidelines that advise against testing unless specific symptoms are present.

The announcement aligns with a broader push by the Trump administration to expand access to testosterone therapies. Health Secretary Robert F. Kennedy Jr. and the FDA have recently moved to relax prescribing regulations, a shift celebrated by influencers who promote the hormone for anti-aging and mental acuity. These claims often diverge from established medical consensus, which typically reserves such treatments for diagnosed hypogonadism.

Political opposition has emerged from within the veteran community and Congress. Sen. Tammy Duckworth and Rep. Chrissy Houlahan criticized the move as a nod to "manosphere" ideologies, noting the irony of Hegseth prioritizing male hormone levels while simultaneously restricting the roles of women in combat. Duckworth, an Iraq War veteran, argued that if the Pentagon is committed to holistic health, it should offer comprehensive hormone screenings for all troops to address systemic issues like infertility, rather than focusing exclusively on testosterone.
